Hannibal Regional's Heart Failure Clinic uses a multi-disciplinary approach to help patients learn to improve their heart’s health through education and counseling for lifestyle changes.
What is the Heart Failure Clinic?
- This program is for individuals with a heart failure diagnosis who would benefit from having extra support from a nurse/pharmacist team.
- Patients in this program will receive one-on-one education about their medical condition and medications, medication management, and close follow-up with the heart failure team to help get their heart failure better controlled.
- You will receive education either through in-person visits or via telephone.
How much does it cost?
- This program is free of charge! Patients do not need insurance coverage to be able to participate in this program.
Who is eligible?
- This program is for individuals with a heart failure diagnosis who would benefit from having extra support from a nurse/pharmacist team.
- Persons who are willing and able to learn more and be more involved in their health would be perfect candidates for this program.
- The service includes a combination of in-person and telephone visits, so patients do not have to live in Hannibal to qualify for this program.
